LCM Athletics would like to thank you for your cooperation. Using these electronic forms will expedite your student’s participation paperwork as well as conserve valuable resources.

YOU MUST HAVE YOUR STUDENT ATHLETE’S STUDENT ID# BEFORE STARTING THE ONLINE PROCESS

* For High School students, you must fill out ALL forms.
* For Junior High School students, you must fill out each form EXCEPT the UIL Steroid form.

* You and your student athlete must fill out and sign in all required spaces on the forms. Forms will not be accepted electronically without everything filled out by the student athlete and you, the parent/guardian. To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box and redo your signature.

* A new physical exam must be filed out each school year for every student-athlete. Physicals must be dated after MAY 10th to be valid for the following school year. A physical will be valid for one school year, regardless of the date of the examination.

* You CAN FILL OUT the PREPARTICIPATION PHYSICAL EVAULATION MEDICAL HISTORY/PHYSICAL EXAMINATION online BUT YOU MUST PRINT AND TAKE WITH YOU TO THE PHYSICIAN. YOU WILL BE UNABLE TO SAVE THE FILLED OUT FORMS TO YOUR COMPUTER. It must be filled out by a physician and hand delivered to ONLY the Campus Athletic Trainer (high school campus) or Sport Head Coach (middle school campuses) before a student participates in any try-out, practice, athletic class, open gym, open weight room, athletic competition, or travels with an athletic team for any purpose. A student-athlete is not considered cleared for participation until this is done.

* Once you have filled out all the information on each page you will have the opportunity to print the document.

* You will receive a confirmation email once the document has been reviewed by the LCMCISD athletic training staff or middle school coordinator.
